MUTARS® Proximal Femoral Replacement & Proximal Femoral Replacement RS

The type of MUTARS® proximal femoral replacement required depends on whether trochanteric reattachment is planned. If so then the RS version is best suited as this option allows the use of titanium alloy (TiAl6V4) trochanteric plates that are screwed to the lateral shoulder of the proximal component and capture the osteotomy.

The system is modular comprising the proximal components themselves, the extension pieces and connecting part – all of which are available in silver-coated options that are ideal where infection is a concern (e.g. if the patient’s immune system has been suppressed by chemo or radiotherapy). Curved cementless and cemented stems plus a range of bolts or screws that securely connect the implant construct are also available.

Description

The TiAl6V4 proximal femoral components come in 50mm and 70mm lengths. It should be noted that in the RS (i.e. the trochanteric reattachment) version both of these proximal component lengths have 127° or 135° CCD-angle options to choose between. All of these implants have a 12/14 morse taper for mating with a femoral head – made from either cobalt chromium molybdenum alloy (CoCrMo), BIOLOX® delta alumina ceramic (Al2O3 and ZrO2) or titanium alloy coated with titanium nitride (TiN).

When either a 50mm or a 70mm proximal component is combined with a stem, this gives a resection length of 80mm or 100mm respectively. Anteversion can be adjusted in 5° increments to optimise stability.

However if extension pieces (40mm, 60mm, 80mm, 100mm) are used between the proximal femoral component and the stem, the reconstruction length can be increased up to 340mm in 20mm steps. Again, here anteversion can still be adjusted in 5° increments to optimise stability.

The 100mm TiAl6V4 connecting part is essentially an extension piece that can be disassembled during a subsequent surgery to facilitate conversion to a total femur if required.

There are nine 120mm long curved hydroxyapatite (HA) coated TiAl6V4 cementless stems (in diameters of 12mm to 20mm) and four 120mm long curved matt-finished CoCrMo stems (in diameters of 11mm, 13mm, 15mm, 17mm) available in lengths of 120mm, 160mm, 200mm and 240mm.

The MUTARS® proximal femoral replacement components have circumferential retention ring features to help keep an optional polyester soft tissure reattacment tube in place during closing.

Alternative options for fixation to the bone include:

  • Diaphyseal implant and connecting part,
  • RS coupling device (30mm) with a RS stem,
  • tapered, cementless stems.
Indications
  • proximal femoral tumour
  • major proximal femoral bone defects including those left behind after revision for failed arthroplasty
  • can be used as a hemiarthroplasty or as a total hip replacement
Materials
  • implatan®; TiAl6V4 according to ISO 5832-3
  • implavit®; CoCrMo according to ISO 5832-4
Coatings
  • TiN-coating optional (cemented stems)
  • implaFix® HA; HA-coating according to ISO 13779-2 (cementless stems)
System Components
  • Proximal femur in two different lengths of 50 mm and 70 mm with taper (12/14), with retention rings for fixation of the attachment tube
  • curved femoral stem in different diameters, cemented and cementless with HA-coating
  • extension pieces
  • connecting part
  • head: CoCrMo, ceramic BIOLOX® delta/forte or titanium alloy with TiN-coating

The treatment can be adjusted with the use of the extension pieces in 20 mm increments. The Proximal Femur allows the adjustment of the antetorsion angles in 5° increments. All components allow the rotational adjustment of 5° increments, too.

Alternative fixation to the bone: with Diaphyseal Implant and connecting part, RS coupling device (30mm) and tapered RS stem as well as tapered, cementless stems.
